I need a medical exemption from licensed physician or medical practitioner.
I have a medical reason for which I can not participate in certain work requirements. My condition is documented from years ago. I can send my medical records and we can have a consultation.
I am working with my normal doctor on this but I cannot get an appointment until a much later date, and I need the exception sooner.
