Medical Consent Form Signature
Budget: $15 – $25 AUD
I need a General Practitioner (GP) to sign a medical consent form on my behalf. The purpose of this document is to legally authorize specific medical procedures or treatments.
The detailed medical information that the GP will be reviewing before signing the form will be provided by me. As such, this will require an in-person consultation where I'll present the necessary information.
Ideal candidates for this job would:
- Be a qualified and practicing GP.
- Have experience in reviewing and signing medical consent forms.
- Be able to maintain confidentiality and handle sensitive medical information responsibly.
- Be willing to meet in person to discuss and review the medical information.
