Medical Coding Expert Needed for Insurance Reimbursement

I had cosmetic surgery from a doctor who does not take insurance. I have a cosmetic rider that will reimburse me 60% of the cost if I can provide the proper documentation with codes. The doctor who performed the surgery provided none, just labels of what was done: neck lift, face lift and tip rhinoplasty. None of it was medically necessary, but as I have stated above I still can receive a 60% reimbursement. I don't even know if third party provided codes would be acceptable. Please let me know and what your cost would be. Thank you.
