I have a backlog of medical claims that must be transcribed into Microsoft Excel with absolute accuracy. Every row will capture patient information only—no billing figures or treatment notes are required this time, so you can focus on clean demographic and identification fields.

You will receive scanned claim forms in PDF format. For each form, simply key the requested patient details into the template I provide, double-checking spelling, dates of birth, policy numbers, and any other identifiers. When the day’s batch is complete, save the workbook and return it to me through our shared folder so I can run my audit macro before sign-off.

Because these are protected health records, you’ll need a reliable internet connection for the secure file portal, plus the discipline to follow basic HIPAA-style confidentiality—no printing, no local backups, no sharing. I expect quick turnaround on each batch (24-hour window) and a running accuracy rate of at least 99 %.

Deliverable:
• One Excel file per batch, fully populated and spot-checked for accuracy.
