Medical Billing for Testing in Connecticut

I need someone that can take 2 Requisition forms for genetic lab testing and see if those codes can be billed in Connecticut.There was a lot of changes that took place late last year so I need to make sure this is possible before starting to send to this lab. The two test are Parkinson-Alzheimer-Dementia NGS Panel,Diabetes-Obesity NGS Panel.
