I need a Doctors note written saying that I was unable to work due to a car accident on 10/20/2021 Till 06/01/2021

Was unable to work due to a seizure I had on 10/24/2021 after a car accident that happened 4 days prior. In addition to the head injury I also regularly had to see a chiropractor throughout this time.
