please I would like you to make me a Template Word format for a Report:
the Report is for Medical eyes Diseases , I want to be able to Edit the Passport number and the name of the Client and type the Type of diseases discovered and the Notes.
it should be an easy job but please only do it if you have information about medical field.
the form should be filled professionally and I should be able to put the name of the institution who make the report and I should be able to edit the date .
the file should be made as tables and paragraphs and one of the paragraph should be the client information like age name address etc..
