Write Medical Student Performance Evaluation

I want to prepare an MSPE for residency application. The work would include writing and designing graphs. In writing, descriptions of excellent work the student did in each department like medicine, surgery, pediatrics, obgyn. Send a sample of two-three sentences on any ONE of the above departments to be considered.
