I need a formally stamped medical report that I can submit to my college. The certificate must be issued on a legitimate clinic or hospital letterhead, signed by a licensed physician, and carry an authentic stamp or seal so the administration has no reason to question its validity.
To be accepted, the report must clearly state:
• The diagnosis and any treatment prescribed
• The doctor’s recommendations (including any activity restrictions)
• The exact duration of absence required
