I’m filing a medical-malpractice action and need a licensed physician who can issue a Certificate of Qualified Expert (CQE). Your primary task is to examine the complete chart, determine whether the defendant departed from the accepted standard of care, and set that opinion out in a signed affidavit that is admissible under state rules.
The records are already organized chronologically in PDF; you will receive them through a secure portal together with my working timeline and interrogatories. After review, I need:
• A written standard-of-care assessment that cites the pertinent entries in the chart and current clinical guidelines.
• The CQE/affidavit, signed with your current medical license and CV attached.
• Availability for follow-up questions or deposition, if the court requires clarification.
