I need help turning full patient records into tight, clinically useful overviews. For every chart I provide, the finished summary must clearly present the patient’s key diagnosis and the course of treatment that followed. Clarity, accuracy, and strict confidentiality are non-negotiable.

• One summary per patient record, written in professional medical language that any clinician can scan in seconds
• Separate headings for Diagnosis and Treatment so information is instantly locatable
• All personally identifiable details kept anonymized to remain HIPAA-compliant
Acceptance criteria
• No extraneous narrative or lab data unless it directly supports the stated diagnosis or treatment choice
• Final document delivered as an editable Word file (or Google Doc if you prefer) and stays under one page per patient
