Help with locating my Assisted Living profile

I am hoping you can help me with a medical records request. I have a full assisted living profile. This profile should be easily available via Sentara Norfolk General Hospital in Norfolk Virginia. However, I am continuously having complications locating the profile when I contact the hospital. It is my awareness that my Assisted living profile should be available via request at any major hospital clerks desk along with other disabilitiy accommodations. I have very extensive medical records available via my Epic emr and Sentara account number which should also provide availibility of my assisted living profile. I am attaching a page from a Sentara record that documents me having an assisted living profile. I am also providing a copy of my credentials. Would your services be able to provide me with a copy of my Assisted Living record/profile cheapily and speedily? The document I am sending will show proof of the record within the section labeled "discharge information". Under "discharge disposition", it will state "Home/Assisted Living". The hospital has iterated time and time again that this does mean that I have assisted living
available. Please note that I have requested from the hospital's record request portal on multiple
occasions to no avail. Thanks.
