Patient needs to correctly code an invoice for neurposych testing for insurance claim

I'm looking for assistance with correctly coding an invoice for neurposych testing for a private insurance claim. I have some of the necessary medical documents and codes, but not all. I'm not sure if I just need help with coding the invoice, or if I require additional help filing the claim.
