During full streamer recovery operation, the starboard deflector had been disconnected from the
streamers and was deployed back out to approx. 450m for recovery at a later time. Later in the
evening the Superwide Winch hydraulics failed resulting in the winch slipping and the superwide on
the starboard deflector to pay out. The vessel speed was slowed to a minimum and the winch break
tightened by hand in order to stop the winch paying out. The winch was then secured with chain to
the deck pad eye & secondary secured with spectron rope to the frame. The hydraulics failed due to
the flushing lines fracturing on the motors, the winch then started to slip, the break not holding due to
drop in hydraulic pressure, then one of the pressure lines for one of the motors blew.
