Program a PDE with boundary conditions in COMSOL

I'm looking for someone who can help me program the following PDE with its boundary conditions in COMSOL, I have tried it but I cannot figure it out, I don't how to program the equations in general form, or coefficient form or in weak form.
I require assistance with this project, and it should run and give the results I already have.
