Deveopement of web containing a Scoring Algorithm

We are a research company that have developed a screening test for Schizophrenia patients. Our test needs a calculator to make a final composite score, based in some simple calculation steps. We have it now developed in an excel sheet, but we would like to have it in a simple web where clinicians could just enter the individual scores and then get the final result as composite score.
The scoring process requires as input a couple of sociodemographic patient features (gender, age and completed education years), and the results of 4 tests administered during a clinical visit. Following a stepwise process of about 4 steps, the scoring provides a final score ranging from 70 to 170 approximately.
We have also a cut-off score that should be shown to the clinician classifying the patient into two categories.
