Market Research Form , I need data on the excel at the end

I would like to start a business of sustainable sugarcane-based takeout food containers, bowls, plates and wooden tableware. I like to research what size containers local restaurants purchase, per month usage, current purchase price, current supplier, their contact info including address, restaurant name, email address, manager or key person of purchasing name, contact no. etc. So I can go to a restaurant in person and get it filled or send them to fill their own and it helps to better understand of their needs and pricing point of view. please let me know if anybody can help. form looks to very nice graphically as well to attract them as a future customer.
