Code a survey into Survey Monkey

I have a 30 question survey in a word document that needs to be coded into Survey Monkey. I have the survey monkey account but I just need the survey coded, I don’t have time to learn and do it myself. It’s a very simple survey with no complicates skip logic. I will provide the word document and the login details for Survey Monkey.
