English-Malay Translation & Survey Entry

I have a set of short survey questions written in English that must be translated accurately into Bahasa Malaysia and then entered into an online questionnaire platform (Google Forms, Typeform, or a comparable tool). The work is split into two clear parts:
1. Translate every question, option, and instructional note into natural, reader-friendly BM while preserving the intent and tone of the original English.
2. Populate the live survey form with the translated text, checking line breaks, character limits, and overall formatting so the form looks polished and is ready to share.
Accuracy and cultural nuance are essential, so I’m looking for someone who is genuinely bilingual and comfortable adapting informal as well as formal language. Your experience with online form builders should allow you to move quickly without needing detailed step-by-step guidance.
Deliverables I need from you:
• A bilingual reference document (English side-by-side with BM) for future edits.
• The completed, publicly shareable survey link, fully tested on desktop and mobile.
