Mailchimp / squarespace expert

I have a Squarespace site with an email sign-up that is supposed to be linked to a list I created in mail chimp. Now all I get is unsaved block form error messages and my sites are not collecting the emails like I want them to. Copy of the error message is below, looking for someone who can assist with Mailchimp email lists and mass emails. this should be pretty basic for someone who knows what they are doing, Please send an estimate.
