Mailchimp Setup (Subscriber Confirmation and First Email Template)

Hi,
We have a very basic requirement to do the following with Mailchimp:
1. Create a simple form that has 3 fields (Name, Surname and Email Address) and provide the code that we can give to our webmaster
2. Create and send a HTML version of the subscriber confirmation email (inside Mailchimp)
3. Create and send initial HTML version of email from Mailchimp (after the subscriber confirms their subscription)
We will need a template made that can be reused over and over again. We want each email that goes out to have the option to view the email in a web page template (using the *|ARCHIVE|* merge tag).
You will already be very familiar Mailchimp and consider yourself as an expert user. When you apply for this role, please make your opening comments say "Easy To Work With Mailchimp Expert Here" at the opening part of your bid.
You will need to use the logo of our website and make the HTML email template look and have the same colours as the main website so that when people get the emails, it will look like its part of the same design family as the main website.
Please don't auto bid on this really straight forward project. Do not put place holder bids. Whatever you bid is the final price you are prepared to do this job for - don't waste our time with placeholder bids. We will ignore all bids that do not follow these very basic guidelines and requirements.
