Magento 1.9->19.4 checkout observer debugging

Recently upgraded from Magento CE 1.9.4.5 to Magento LTS 19.4.15
Checkout Helper override not working anymore, was working in 1.9.4.5.
Need someone to help debug this issue and get my checkout working again.
Initially you will need to provide steps to follow to identify issue without access to code or server.
This will be a joint debugging/modifying effort.
You will not have access to my server.
