Magento Shipping Extension

We currently use multi shipping rate extension which has been set up in admin with all rates etc. We have set up shipping classes with multiple options. For example if shipping a 3/4 snooker cue to Italy there are 2 rates set in admin. 2-5 Days $40 or 5 days + $17. At checkout client is only seeing $40 option. We require customer to see both shipping options. see example screen shot below.
