Extracting medical test reports from PDFs

I want to extract medical test details from pdfs. Extracted test details will be a list of test names, its result, unit and min-max range. Like if in the pdf file, hemoglobin test is there and its value is 12.3, mg/dl unit and 11.5-14.5 min-max range. So in the output there will be a list of these four parameters. I have also attached a sample pdf file here. Please note that there is no fix format of pdf. Report format is not fixed. I have more than 15 pdf files to test the algorithm. And I need more than 90% of accuracy. You can message me for more details.
