Logo for twitch creator.

I'm gamer and streamer at twitch. My nick is associated with hockey even it hasn't much in common with what I'm doing, but it has a reason. I'd like to have logo of a hockey player in colours and style of Hotline Miami (pattern in uploaded files). I'd also like two logos. First one - hockey player from the front and his face overshadowed by his helmet with his visor and hockey stick hold over his shoulder (left handed from this position). Second one - from behind, again hockey stick over his shoulder (right handed from this position) and with jersey number ''22''. Both versions also include text where is typed ''Hokejový Svaz''. I'd also appreciate alternate version with ''Hokejový'' only. Thank you.
