xcp-ng restore - xenserver restore VDI_IO_ERROR(Device I/O errors)

when I am trying to restore xcp-ng / xenserver virtual machine using xenorchestor it gives me following error
VDI_IO_ERROR(Device I/O errors)
backup vhd file is there but it is not restoring - can anyone help?
