FusionPBX - help configure NAT / Gateway

I've installed FusionPBX on Azure.
I can make calls between extensions but calls to and from my gateway is failing. Help diagnose and troubleshoot.
The same configuration on a VM that I have hosted somewhere else works fine as the VM has a native public IP address. The azure one has both private and public IP. So suspect some sort of NAT issue. This would be a quick fix for someone who knows what they are doing.
