Hair question survey

I am looking for experts in survey design to create a hair question survey for me. I don't have any existing questions for the survey, but now need help putting a hair quiz together that will take 5-15 minutes for users to complete. The aim of the survey is to get insights into customer satisfaction, product preference, and demographics. It is important for the quiz to be user-friendly and engaging enough for the user to complete within the designated time frame.
