Terms and conditions agreement for App users

I want to launch a mobile App where patients will be uploading their personal information along with health information, all type of medical records, prescriptions, security numbers, credit card information, National ID number etc. In the terms and conditions I want to ensure their personal data privacy. At the same time I want the permission to utilize data and other relevant information and share that to any third party too without disclosing any personal information.

The agreement will cover the current laws and policies in Bangladesh.
