Court Document for Civil Case Removal

Looking for somebody who is a lawyer or extremely experienced with USA court system. Need to have somebody create a court document to remove a civil case that was filed against me years ago. The case was through a business i partially owned and i was amended on the case. I will provide a sample of what the court provided.
