I have a collection of patient records that must be distilled into concise, legally defensible summaries. Each summary must capture four critical elements—diagnosis, treatment plan, complete medical history, and current or past medications with precise dosages—so the documents can stand up to scrutiny in a legal context.

All source notes will be supplied as scanned charts and EMR exports; you will return a clean, well-structured PDF (and editable format) for every case, written in clear, objective language that aligns with standard medico-legal terminology. Accuracy, strict confidentiality, and adherence to HIPAA best practices are non-negotiable.

Deliverables:
• One summary per patient record (1-2 pages each)
• Embedded page references to original file locations for quick cross-checking
• Final file set delivered as labeled PDFs plus editable DOCX

Acceptance criteria will be met when every summary contains the four required sections in the order listed, reads coherently without medical jargon overload, and has been double-checked for factual consistency against the source material.
