Housing Association Access & Safety Complaint

I need assistance drafting an official complaint letter to my housing association. I’m room bound due to no accessibility in this flat.
Specifically:
causing me to be confined to a single room for over three years now. If there was a fire I could not get out.
I have lived in one room for 3 years. I fall often trying to get to the commode. We r socially isolated due to these circumstances.
My wife is also disabled and her heath is impacted due to having to do everything.
We was told we would be moved immediately if we were band A we got to band a and nothing
Both our mental health and physical health have been greatly impacted and has deteriorated because of this housing situation only.
We r both on anti depressants now so
