Give me Note Sheet template to be used in Indian Government offices

I am looking for a freelancer who can provide me with a Note Sheet template to be used in Indian Government offices. The template will primarily be used for Official Record Keeping. I am open to suggestions regarding formatting requirements, however, I do require a Signature Space on the template. The ideal freelancer for this project should have experience in creating templates for official record keeping and a strong knowledge of Indian government office procedures.
