Form 7A Completion & Service Help
Budget: $750 – $1,500 CAD
I am taking Dr. Oren Amitay — a well-known Toronto psychiatrist — to Small Claims Court in Ontario, under quantum meruit. My Form 7A is already started and the claim statement is partially written, but I want to be absolutely certain everything meets court requirements before I file.
What I need from you
• Review the entire Form 7A for accuracy, missing details and overall compliance.
• Advise on organising and labelling the supporting evidence I must file: receipts/invoices, email or text communications with Dr. Amitay, plus a document that compares his original manuscript with the version that existed when I stopped working on it.
• Explain, step by step, how these exhibits are physically or electronically attached when I submit the claim.
• Guide me through proper service of the claim, including locating or instructing a licensed Ontario process server and preparing the Affidavit of Service.
Acceptance criteria
– Finalised Form 7A ready to print or e-file.
