ADA Court Forms Assistance Needed

I have to submit an Americans with Disabilities Act accommodation request to the court and the step that is holding me back is actually completing the paperwork. I already know the accommodation I will be asking for; what I need is someone who understands court-specific ADA procedures to walk me through the forms that fall outside the standard “Reasonable Accommodation Request” or “Medical Certification” templates.

Here is what I’m looking for:
