Laravel Expert to add new Section in our already working system

We need a Laravel Expert.
To add a new Section to our already developed Laravel system.
This new section it's a "Form" where depending on the User Level can Access it or Not.
The Form will add new data to the DB, and to users profiles fields.
It's needed to make a new Table in the DB to save the data and interrelation with already users DB data.
Some of the data of the Form will be auto-loaded depending on one Field, and the auto-loaded data can be edited for the Form only (not modifying user DB data only Form DB data).
The Form contains some fields that needs validation, like number and type of characters/letters/numbers.
Each Form will have 4 Users linked to it, each User linked to it, will have a unique Position Type in each Form.
We need to add the option to Edit and Delete data, only to "Creators" of the data or Upper Level users in hierarchy.
In the already done (in our system) "listing of users", we need to Show the total Form submitted by each user, only for Admin users.
Important notes:
1.- We don't work with advance payments
2.- We need finished in maximum 5 days
3.- You will need to dedicate time (1 hour more less) to understand our current system
4.- We are looking for Freelancer for the long cooperation as we have several requests
5.- We work only for pre-quote total, we don't work by the hour, or "common sense" extras, if you see something will be needed in our explanation, you need to advise before accepting work
