I need a robust, high-precision clinical database that will fully automate the chemotherapy ordering workflow, acting as a safety bridge between diagnosis and drug administration. The system must run only inside the hospital network, with no data ever leaving the local environment.

Current deployment will sit on a single workstation (i5 11th Gen, 16 GB RAM) yet the codebase should be cleanly structured so it can later scale across the entire hospital. For the data layer I am comfortable with SQL Server, but MS Access with solid VBA is acceptable if it fits better. Either way, performance, fault-tolerance and an audit trail are mandatory.

Interface preference: I would like this delivered as a Laravel project, giving staff a familiar, modern GUI while still letting me keep everything on-premise.

Type of application: the core is an order system—specifically a product-ordering module that captures drug protocols, calculates dosages from diagnosis data, applies safety checks, then produces the final pharmacy order.

Key requirements
• Full CRUD around patient regimen templates, drug catalogues and dosing calculators
• Real-time validation against contraindications and cumulative dosing limits
• Secure role-based log-in plus tamper-proof logging of every change
• Installable on a local desktop or internal IIS server without external dependencies
• Straightforward pathway for future multi-workstation roll-out (e.g., modular services, REST endpoints)

Deliverables
1. Source code (Laravel/PHP, .NET or VBA components as agreed)
2. SQL Server (or Access) schema with seed data and migration scripts
3. Installation guide for a stand-alone workstation
4. Brief user manual highlighting the ordering workflow and safety checks

I will test the build against real-world oncology protocols; acceptance is complete when the system installs locally, processes a full chemotherapy cycle without errors, and logs every activity accurately.
