I’m looking to build a doctor map that looks and works (almost) exactly like this one

It would need to be searchable in the same way this one is.
It would also be great to be able to add a medical practice through a user interface that holds entries for approval.
I’d like to hide this map behind an opt-in gate, so it’s not available unless users have provided their email.
I might be interested in a contact form, within each directory listing, which allows patients to send an inquiry to the doctor in the listing.
