Fix Bugs in PHP/WordPress Portal

I run a bespoke portal that marries WordPress for its CMS layer with a hand-coded PHP backend. Two pain points need immediate attention:
• Front-end display glitches: several forms and input fields render incorrectly—labels overlap, some fields vanish on mobile, and inline validation messages float out of place.
• Database hiccups: queries tied to those same forms intermittently time-out or return malformed data, hinting at schema or query-logic faults.
You’ll work on a staging copy, with SSH and wp-admin access plus a clean Git workflow. The stack is PHP 8.1, MySQL 5.7, vanilla WordPress (no page builders), and a sprinkle of jQuery.
Acceptance criteria
1. All forms display and validate properly in the latest Chrome, Firefox, Safari, Edge, and on iOS/Android browsers.
2. Database operations run error-free; submissions store and retrieve accurate data with no warnings in the logs.
3. A light regression pass confirms nothing else breaks, and you share a concise change log within the pull request.
