Create a static web page

I want a web page that will reproduce the look of the attached files.
Page must be built with html and javascript/jquery.
Animation required :
A click on one item in "Add a goal" section on top right should create a new point in the timeline based on year of the goal.
Information are not saved. They are keep temporaly in the browser.
No connection to backend database or rule engine.
