input form jquery validation

i need to make on an input form, jquery validation, after accept bid i will send to you html
form have 6 fields, firstname lastname,email, 2 doc uploads and terms and condition
need to check if name is empty or not and to show error bellow if is empty, on email same if empty and if is valid email , on doc upload to check file extension, and on terms and condition also if it empty or not, and if form is completed without errors to make loading spinner 1 sec and after show success message
