Searchable database for gps images of public works issues with many searchable fields and report building.

I am looking for a developer to create a searchable database for GPS images of public works issues. The database should have the following fields: location, issue description, and tailored fields to suit this project. Attached is a more detailed selection of detail, an older version we used to use which we are refining this from and one sample offender report.
The database should be accessible on both web and mobile platforms.
Additionally, I would like the database to have the ability to generate both standard and custom reports.
