contact form for joomla -- 2

need a contact from for Joomla where user has to fill 10 fields for example: each field gets 10% if it is filled when sent in backend they system should calculate how many fields filled: if 30% and less an anaswer will be sent|if 50% answer 2 will be sent if 70% answer3 will be sent if 100% fileld answer 4 will be sent
