Fix External Site Opt-In Form Code For Joomla AcyMailing List

Hi. I have a new, very basic Joomla website with the AcyMailing plugin (Enterprise version). My external websites have a form to subscribe people to the AcyMailing list. The code is all there, but I need help to make the form work properly. Must be a line or two that needs fixing. Captcha needs work with it, too. Thanks
