Convert Joomla 3 Module to Joomla 4 , and php 7.4 to php 8.1

We would like to have a Joomla 3 php module modified so that it can be installed on Joomla 4.
The module is a small module that allows us to use a php form on a page. The php form is in 7.4, we would like to be able to use it in 8.1.
We have attached some files from our J3 module.
