A web application to support the conduct of studies. Specifically, to record the daily experience of the user in the context of longitudinal studies of the evaluation of interactive systems

I have a project which involves creating a web application to support the conduct of longitudinal studies. Specifically, the web application should be able to record a user’s daily experiences of an android mobile application in the context of longitudinal studies of the evaluation of interactive systems.
The web application needs to have a feature for user registration and login. The users (1) should be able to fill in a form, every time they want to create an evaluation form for the specific android app they want to evaluate. So the users (1) are going to create dynamically evaluation forms for the android apps to be evaluated with a unique ecrypted link for each evaluation form. Each user (1) could have different android applications to be evaluated with different evaluation forms. When the users (2) fill in the evaluation form, the results of the users' (2) feedback should appear in a dashboard which can been seed only by the user (1).
Ideally, the application should be made for desktop use and shoud be responsive.
User (1): The users are going to fill a form, create dynamically evaluation forms and see their results.
User (2): The users are going to fill the evaluation forms.
