I need some good in javascript and php for a webconnect/php login page and linking

I have design ready, its a three page website, You will be doing some minute html fix to the first page, on the second page, you will do correction that appear on the screenshot attached, and on the last page you will do what in the video attach to this post, please kindly make sure you watch the 1 min video and look the screenshot before you respond else i will not be happy repeating myself all over again..
