I need to have an inspection report generator in power app asap

I need to have an site inspection report generator in power app that link to my sharepoint folder.
The first screen would be project name selection which link to my sharepoint folder. (my sharepoint having each folder for each project.), Date & Time, Location, which are Auto key in or 1 click to insert, and Reporter which is drop down list.
The element title i would like to have in the report detail screen is adding site photo and its description.
The third screen will be attendees, remarks etc.
Then review and have 2 type of report generated in PDF, 1 is simple PDF which showing project until site photo and description, another detail PDF is including the last pages which having attendees and remarks etc. Both PDF should have our title head as header, project number and page number as footer in every pages.
