multiple platform app

I need a multiple platform app with JavaScript frame work such react native. The user after downgliding the app should agree to a consent form and after agreeing, they answer 30 multiple choice questions only once, and then they should answer 60 multiple choice questions. After that they gets notification twice a day to answer 15 multiple choice questions (they have to answer the same 15 question twice a day). Notifications are just for reminders and they can answer the 15 questions anytime of the day. User has to do this for 14 days. After 14 days they have to answer the same 60 questions that they initially answered. All the answers for each user should be in csv file and I need that csv file. Answers for each question should have timestamp. I also need user authentication.
