3 levels cascade inputs with Selectize.js

i need a javascript hand with this:

I need to apply Selectize.js to a form like the linked one (3 levels cascading dropdown).
It can be done in jquery and selective or pure js, but i need to enter the select values like the example, array notation (no ajax load).
The winner should be give me a jsfiddle with working example, with well written code, and the submit must work.
